Consent and confidentiality

Before you attend any type of appointment you will be required to confirm your informed consent. At the beginning of any appointment your consent will be confirmed (verbally or signed), and again at the end of the appointment.
You have the right to refuse your consent, however you should be aware that if you decide not to give your consent, your employer may have to make decisions about you based on the information available to them without the benefit of a clinical opinion. This could potentially result in you not receiving modifications or adjustments to support you in your role.

The report

You can request a copy from your employer, see a copy of the report at the same time as it is sent to your employer or request a copy before it is sent to your employer. Please note that only factual inaccuracies and information subject to revoked consent may be amended within the report if requested. We cannot amend our clinical judgement and reasoning.

Call recording

All calls made to and from Innovate’s Case Managers are recorded for training and monitoring purposes.

Unable to attend?

Please be advised that should you be unable to attend the confirmed appointment, we would kindly ask that you notify us within 48 business hours of the appointment, as a failure to notify us within this time frame could incur a charge for the appointment that the referrer would be liable for.
